This document asks for information about the non-custodial parent, including details about homes addresses, medical insurance, job locations, and pay cycles. You may also receive a similar request for information from a different state that is pursuing income withholding against one of your employees. Complete the information in the same way that you would for Tennessee and return the form to the requesting jurisdiction.
